IDENTIFICATION OF TYPICAL MISTAKES IN COMMUNICATION BETWEEN A DOCTOR AND A PATIENT IN SIMULATION CONDITIONS AND WAYS TO OVERCOME THEM

Abstract

A high-quality exchange of information between a doctor and a patient is a necessary condition for their effective interaction. The communication skills of a doctor, including at the stage of his formation, must be constantly improved, including through their constant development. The authors present the results of a study of evaluating the effectiveness of simulation courses aimed at improving the communicative competence of students of the medical faculty. In the course of a week-long course with input and output control of students’ knowledge, it was shown that the identification of typical communication errors in simulation conditions with their subsequent correction in practical classes is an effective method for ameliorating the communication skills of future specialists. The introduction of simulation practices in the training of medical students makes it possible to form the competencies necessary for this, which in the future can significantly increase both compliance and immediate and long-term results of patient treatment.

ANALYSIS OF THE POINT-RATING SYSTEM APPLICATION AT KURSK STATE MEDICAL UNIVERSITY

Abstract

A form to control the level of university students’ knowledge is a point-rating system. It is based on motivated, systematic work of both students and university teachers. The information transparency of the point-rating system makes it possible to more objectively assess the level of students’ knowledge and motivate them to deeper prepare for the lesson. The article reveals the basic principles of the point-rating system application, and its advantages over the traditional approach to monitoring students’ learning of educational material as well. The rating system has been introduced at Kursk State Medical University since 2016. The authors of the article carried out a survey of fifth-year students of the medical faculty in order to identify their attitude to the current point-rating system. The students’ comments concerning the advantages and disadvantages of the system have been also analyzed. There have been made some conclusions saying that there is a need to revise some paragraphs in the current regulations to optimize the process of adaptation and further successful functioning of the point-rating system at the university. It has been established that this system adequately assesses the students’ educational achievements. The guiding principles for changes and improvements in the rating system should not be based on the students’ opinions only, but on the experience of other medical universities and the teachers’ opinions as well.

ASSESSING THE PROFESSIONALISM OF PHYSICIANS BY PATIENTS: TRANSLATION INTO RUSSIAN, ADAPTATION AND VALIDATION OF THE IPAMP QUESTIONNAIRE

Abstract

The assessment of medical professionalism does not lose its relevance, since it is associated with the assessment of the quality of training of future doctors and the provision of medical care to the population. Existing tools for assessing the professionalism of doctors in the Russian Federation rarely take into account the opinion of patients or their legal representatives. The International Patient Assessment Tool for Medical Professionalism (IPAMP) includes 11 items and is designed to assess the professionalism of trainees and doctors from the patient’s point of view.

Purpose. Translation into Russian, adaptation and validation of the Patient Assessment Tool for Medical Professionalism (IPAMP).

Material and methods. A cross-sectional study was conducted on the basis of the Arkhangelsk Children’s Dental Clinic of the Arkhangelsk Region to test the reliability and validity of the Russian version of the questionnaire (R-IPAMP). 336 legal representatives of patients (parents) were anonymously interviewed, who assessed the professionalism of 7 dentists with a work experience of no more than 3 years, under the age of 30 years.

Results. Intercultural translation and adaptation of IPAMP into Russian was carried out. Structural and criterion validity of the assessment tool are determined. The Cronbach’s alpha coefficient for 11 items of the questionnaire was 0.814, which indicates good reliability of the assessment tool, internal consistency of the questionnaire questions and their homogeneity. We analyzed the response data of 336 legal representatives of patients who highly rated the medical professionalism of young doctors (4.56±0.04).

Research limitations. Standardization of the questionnaire requires further study of its convergent and discriminant validity, retest reliability, and factor analysis on a larger sample of respondents.

Conclusion. The proposed version of the Russian version of IPAMP is a valid and reliable tool for assessing medical professionalism on the part of patients and can be used at all stages and levels of medical education, as well as in practical healthcare to receive feedback from patients or their legal representatives in order to correct the educational process and improve interactions in the doctor-patient system.

ADAPTATION OF THE EDUCATIONAL PROCESS IN ADDITIONAL PROFESSIONAL EDUCATION

Abstract

One of the features of additional professional education is the need to adjust the educational process to the constantly changing requirements of the contingent of students. The paper presents an analysis of many years of experience in the ongoing adjustments of the educational process of postgraduate education cycles at the Department of Medical Informatics and Physics of the North-Western State Medical University. It is shown that the most important condition for successful adaptation of the educational process is the creation of prompt, reliable and properly organized feedback channels, such as a preliminary interview with students and their final survey. Possible ways of solving emerging issues in the general and current organization of teaching process related to identifying and taking into account the suggestions and wishes of students to adapt the process to the needs of students and improve its quality are proposed.

ORGANIZATIONAL AND METHODOLOGICAL ASPECTS OF THE ONLINE LECTURE COURSE IN CHEMISTRY FOR FOREIGN STUDENTS OF THE URAL STATE MEDICAL UNIVERSITY

Abstract

There are analyzed the features of the lectures organization on the example of the basic discipline «Chemistry» for foreign first-year students of the Ural State Medical University (USMU), studying in the specialties «General Medicine» and «Dentistry». The distance form of the lesson, the English-language presentation of the material determines a number of special organizational and methodological approaches to the process of lecturing. The USMU electronic environment allows the teacher to give lectures in synchronous mode (online) on the MS Teams platform, using all the technical capabilities available there. Video files with recordings of lectures and pdf-presentations were posted weekly for foreign students, access to it was opened on a permanent basis using the Internet. Slides of lecture pdf-presentations were maximally filled with graphic and textual information with concise and clear wording. The lecturer observed a slow pace, semantic pauses and accents in speech, emotionally colored intonation. Distant testing was introduced at the end of each lecture to increase the level of attention of students and involvement in the educational process. Analysis of the results of intermediate controls, both at lectures and at practical classes, showed that more than 60% of students completed tasks with an excellent mark. The results of testing on each topic of the lecture and the results of intermediate controls in practical classes on the relevant topics correlate with each other, the relationship is linear, positive (r=0.7).

Data from anonymous lecture quality satisfaction survey of students will be taken into account in the future to improve teaching methods.

ADAPTATION DESIGN OF THE FOREIGN LANGUAGE COURSE FOR FUTURE OSTEOPATHIC DOCTORS

Abstract

The authors of the research propose solutions to the challenges in development of novel curricula in the framework of medical education. In 2022–2023 academic year I.I. Mechnikov’s NWSMU launched an Osteopathy curriculum considered unprecedented among Russian medical universities. To build second language communication competence at future doctors-osteopaths, we theoretically defined with PRISMA analysis the efficacy of pedagogic adaptation design method for the development of modern educational programmes, furthermore, we defined it. The authors forecasted that pedagogical adaptation design method is able to minimize risks of inefficiency of novel programmes, which resulted in verification during implementing and monitoring activities. We detected that planning the learning outcomes, teaching-and-learning aims, contents of the discipline are to be grounded on average student profile, osteopath-to-patient communication practice, teaching staff expectations for the programme implementation. The current paper presents the outcomes of the questionnaire on students’ profile, osteopaths’ – on how to build the content of the discipline, teaching staff – to describe expectations from the discipline of foreign language, which determined communication material, aims and learning outcomes of the discipline. In terms of pedagogy, the researchers had to identify the most efficient format of teaching and learning activities. In accord with the results of students’ satisfaction monitoring and academic performance during COVID‑19 pandemic restrictions, the researchers proposed to apply the hybrid educational format. In conclusion, the article provides outcomes of students’ satisfaction with pedagogical adaptation method and its prospects.

INFORMAL MENTORING BY “KHASE DOCTORS” AS A WAY OF TRANSLATING THE VALUES OF СIRCASSIAN CULTURE IN THE PROFESSIONAL DEVELOPMENT OF YOUNG DOCTORS

Abstract

Changes in medical education and public health are bringing human resources issues up to date. A promising solution is the development of the institution of mentoring. Consideration and research of positive examples can help young medical professionals in their professional journey. The aim of the research study was to show the positive experience of mentoring “Khase doctors” as an example of professional development, formation and adaptation of young doctors through the transmission of values of сircassian culture.

Material and methods. A qualitative sociological survey of ten young doctors who underwent mentoring.

Results and discussion. All respondents noted positive experiences, improved communication and communication skills, theoretical training and professional medical skills.

Conclusion. Mentoring can be seen as a model for education, professional growth and development of young doctors in health care.

DESIRABLE TYPES OF FUTURE PROFESSIONAL ACTIVITY OF MEDICAL STUDENTS

Abstract

The article presents the results of a sociological study on the preferred types of future professional activities of medical students. The purpose of the study is to identify the specifics of the professional choice of various typological groups of medical students. The study was carried out among students of the Volga Research Medical University using an online survey. The study showed that students of the pediatric and medical faculty would most like to work in a prestigious medical institution – a solid public or private one. Dental students most often reject for themselves public administration that is incompatible with commercial activity. They are less interested than others in working in a medical institution of the regional center, scientific or teaching activities, as well as working in a district hospital. Future pharmacists are also committed to commercial activities and often would like to have their own business. In general, the work orientations of only students of the Faculty of Medicine and Prevention correspond to the profile of training. It is these students who are largely oriented towards future activities in the field of public administration.

The analysis of students’ assessments of preferences for future professional activity was carried out using multidimensional data analysis, which made it possible to identify 4 types of student attitudes: 1) medicine; 2) clinic; 3) science; 4) business. Each of these types contains its own set of characteristics, which depends on the faculty and course, student performance, motivation for choosing a profession and where to get it.

DEVELOPMENT OF DIGITAL COMPETENCIES OF MEDICAL WORKERS

Abstract

This article discusses the importance of developing digital competencies of medical workers in the context of the overall digitalization of Russian healthcare. It focuses on the use of digital skills in the daily practice of healthcare professionals and their relevance in the current and future clinical practice. The issues of the influence of artificial intelligence on medicine and the role of the doctor in the future are discussed.

FEDERAL INNOVATION PLATFORM IN THE ACMEOLOGICAL DEVELOPMENT OF MEDICAL UNIVERSITY TEACHERS

Abstract

Introduction. The search for a model that promotes acmeological development, integrating formal and non-formal training of teachers with advanced training, is relevant. The article is aimed at finding out the satisfaction of teachers with a new format for improving pedagogical qualifications at the School of Pedagogical Excellence of the Volgograd State Medical University. The involvement of teachers in pedagogical creativity within the framework of the federal innovation platform is considered.

Material and methods. A continuous research method was implemented, students of advanced training courses were interviewed without regard to specialty and experience, the information collection tool was anonymous questionnaires using a Google form, validated by similar surveys by institutions of additional education in Volgograd. Information analysis methods: generalization, analysis of information on selected indicators of the questionnaire, the consent of the respondents was obtained on the basis of applications when registering for advanced training courses.

Results and discussion. Data is presented that indicates the satisfaction of teachers with the new format of teaching at the school of pedagogical excellence. The result of formal and informal advanced training of teachers was the creation of new projects, creative participation in event-based education.

Conclusion. The model of the school of pedagogical excellence, focused on the acmeological development of teachers of a medical university, was developed and tested at the Volgograd State Medical University in the course of the federal innovation platform. The results obtained make it possible to recommend the experience of a new format for improving pedagogical qualifications to colleagues from other medical universities.
