THE APPLICATION OF THE MULTIMEDIA MANUAL “THERMOMETRY” IN TEACHING OF PROPAEDEUTICS OF INTERNAL DISEASES: THE FIRST RESULTS

Abstract

Introduction. The development of digital education and the introduction of digital pedagogy and didactics is limited by the lack of sufficient scientific research and evidence base for their effectiveness. One of the key steps to overcome existing problems and barriers may be implementation of various educational technologies, including e-learning tools, into the educational process.

Aim – to evaluate the effectiveness of using the multimedia manual “Thermometry” in teaching students of a medical university in the discipline “Propaedeutics of internal diseases”.

Material and methods. A multimedia manual “Thermometry” has been developed. It contains theoretical material, examples of graphs of temperature curves, test tasks and clinical cases. In order to evaluate the effectiveness of this educational multimedia manual, a study was conducted, during which 3rd-year students in the process of mastering the topic “Thermometry” were invited to use the abovementioned manual in addition to the main literature sources. The study included 215 students who were divided into two groups. Group 1 comprised 111 students who, along with traditional education, were offered to use a multimedia manual. Group 2 comprised 104 students who studied according to the traditional method. At the end of the lesson, students of both groups were asked to take an electronic test. At the end of the test students were asked to fill in a survey devoted to the quality, benefits and effectiveness of the multimedia manual. Statistical processing of the obtained results was carried out by MedCalc program using nonparametric statistical methods.

Results. Students of the 1st group showed 15.4% more correct answers, compared with the students of the 2nd group (p<0.0001).

Conclusion. The data of the student survey based on the results of the application of the multimedia manual allow us to conclude that the overwhelming number of students enrolled in the study determine the multimedia manual as a high-quality, useful and effective didactic teaching tool.

AN ENTRANCE EXAM IN CHEMISTRY BY MEANS OF MOODLE

Abstract

The paper presents the experience of preparing tests for the internal entrance exam in chemistry at Donetsk State Medical University conducted by means of distance educational technologies on the Moodle environment. The assessment of the test quality using built-in statistical analysis module showed the reliability of the developed scenario of the entrance test. Using the facility index and discriminative efficiency, weak tests were identified and recommendations were made to improve the quality of the tasks being compiled.

EXPERIENCE IN THE DEVELOPMENT AND TEACHING OF THE DISCIPLINE «PHYSICAL FOUNDATIONS OF DIAGNOSIS AND THERAPY» AT A MEDICAL UNIVERSITY

Abstract

Dynamic changes in the scientific, informational and logistical bases of healthcare affect the requirements of professional standards for specialist doctors introduced by the Ministry of Labor and Social Protection of the Russian Federation. The transition to the Federal State Educational Standards of Higher Education (FSES HE) of the III generation in various areas of medical training has led to the emergence of new disciplines in medical universities.

Diagnostic and instrumental examination methods are an obligatory category of general professional competencies for a general practitioner and a pediatrician. The task of forming the competencies specified in the educational standard of future doctors requires comprehensive fundamental training, including knowledge of the physical foundations of diagnostic and therapeutic methods. The study of these fundamentals in the framework of the discipline “Physics, Mathematics” is not sufficient. The task is solved by introducing a new discipline «Physical foundations of Diagnosis and Therapy» into the educational process of the Rostov State Medical University of the Ministry of Health of the Russian Federation. This discipline consists of one module, studied by students for one semester and taught in the 3rd year of training of medical practitioners and pediatricians. The module was developed by the teachers of the Department of Medical Physics, Mathematics and Information Technology. A survey of students was conducted, feedback on the optimization and implementation of the educational process was analyzed. The methods of forming the skills set by the implemented general professional competence are defined. The objectives of the discipline are: mastering the physical basis of modern diagnostic and therapeutic methods and factors affecting the patient, gaining practical skills in working with medical equipment, as well as studying the use of high tech appliances in medicine and healthcare.

The experience of fundamental training of medical students for the implementation of general professional competence related to the use of diagnostic methods and medical devices provided for the provision of medical care can be used in other medical universities.

THE RELATIONSHIP OF A DOCTOR’S COMMUNICATION SKILLS WITH DISTORTIONS AND LOSSES OF INFORMATION AT THE STAGE OF COLLECTING COMPLAINTS AND ANAMNESIS (USING THE EXAMPLE OF PRIMARY ACCREDITATION)

Abstract

The study of the effectiveness of physician communication skills has been a relevant area in Russian reality for the past twenty years. Studies by foreign authors prove that clinical outcomes are associated with physician communication skills and contribute to the formulation of diagnostic conclusions based on the collection of complaints and anamnesis. In domestic medical practice, it is recognized that anamnesis collection is a key method for making a diagnosis, but there are not many studies devoted to assessing the relationship between communication skills and diagnosis. The authors, using the example of graduates of the medical and pediatric faculties passing the communication station “Collection of complaints and anamnesis” as part of primary accreditation, analyzed communication errors, losses and distortions of information when a “standardized patient” communicates his complaints and symptoms to an accredited physician. It was revealed that graduates of the medical faculty reliably more often identify all patient complaints. It was proven that the number of complaints identified through a closed question affects the distortion of information by the physician. The relationship between the communication skills “screening“ (identification of additional circumstances of the disease/condition) and “summarizing“ (repetition by the doctor of information received from patients) with the identification of all patient complaints has been established. It was found that the physician’s use of effective communication skills “screening” and “summarizing” reduces his use of the ineffective skill “closed question”.

PROBLEMS OF HIGHER MEDICAL EDUCATION. DREAM COME TRUE AND HOW TO STUDY (RESULTS OF QUESTIONNAIRE SURVEY OF STUDENTS OF MEDICAL UNIVERSITIES IN RUSSIA)

Abstract

The aim of the study was to obtain objective information, usually not taken into account in official documents, which could possibly help to improve educational outcomes and reduce unreasonable costs to the state in training medical personnel.

Material and methods. An online questionnaire was jointly developed by students and teachers. 454 students of medical universities and medical faculties from different cities of the Russian Federation participated in the questionnaire. The paper discusses the issues of USE score at admission, pre-university history of students, education and work of their parents, on what depends the success of studies, issues of study load and compliance of university requirements with capabilities and abilities, as well as applicants’ expectations. Special attention in this paper is paid to students’ assessment of the quality of education, including theoretical and practical education, self-assessment of confidence in practical knowledge on mannequins, simulators and humans. In addition, the issues of extracurricular activities of young people and aspects of future work were discussed.

Conclusion. 1) In order to increase the efficiency of higher medical education and optimise the financial resources spent on it, it is necessary to develop ways of implementing personal trajectories in university education. 2) It is expedient to intensify the implementation of youth policy aimed at strengthening the civic position of students. 3) The high level of student dissatisfaction with the practical part of education, together with low self-assessed confidence in demonstrating medical and pre-hospital skills, necessitates a review of the structure and geography of internships. 4) The high level of students’ dissatisfaction with the practical part of education.

PRACTICAL EXPERIENCE OF USING THE QUIZ IN TEACHING PATHOLOGICAL ANATOMY AT A MEDICAL UNIVERSITY

Abstract

Improving the quality of education for medical students is an urgent problem for every modern medical university in Russia. To solve it, various methods of gamification of the educational process are increasingly being used. The article presents the experience of using a quiz game in the classroom on pathological anatomy among students of medical and pediatric faculties.

ASSESSMENT OF RESIDENTS’ KNOWLEDGE OF ANTIMICROBIAL THERAPY OF RESPIRATORY TRACT INFECTIONS IN CHILDREN

Abstract

The practice of prescribing antimicrobial drugs for respiratory tract infections in children still raises many complaints.

Аim of the study – to assess residents’ knowledge of antimicrobial therapy for respiratory tract infections in children.

Material and methods. During an organized series of lectures and seminars on antimicrobial therapy of respiratory tract infections in children, entrance and final testing of residents was conducted. The test tasks consisted of 10 questions with five possible answers, one or more of which were correct. The maximum score for an answer was 100 points. A result of 91–100 points was assessed as an excellent answer, 81–90 points – as good, 71–80 – as satisfactory, less than 70 points – as unsatisfactory. Comparative characteristics of differences in answers for the initial and final testing were carried out using the χ2-test with Yates correction.

Results and discussion. The residents’ knowledge of antimicrobial therapy of respiratory tract infections in children was assessed as satisfactory [the average answer score was 71.5 (out of 100 possible)] based on the results of the entrance test. Only 10 (37.0%) residents gave excellent and good answers, the answers of 15 (55.6%) residents were satisfactory, and the answers of 2 (7.4%) residents were unsatisfactory.

Based on the results of educational events, the average score of residents’ responses increased by 11.75 (14.1%) points – to 83.25 points (out of 100 possible). The results of the answers were regarded as good. At the same time, 20 (74.1%) residents gave excellent and good answers. The results of the initial and final testing reached statistically significant differences according to the χ2 criterion, which indicates that the educational material has been mastered.

Conclusion. The authors identified the problem of insufficient development and survival of knowledge among residents regarding antimicrobial therapy of respiratory tract infections in children. According to the authors, active learning methods using the online platforms AMRmap and AMRbook allow optimizing the educational process.

DEVELOPMENT OF INDIVIDUAL LEARNING PATHS IN ADDITIONAL PROFESSIONAL EDUCATION

Abstract

The paper is devoted to identifying problems and tasks requiring solution that arise when developing individual learning paths in additional professional education, using the example of teaching modern information technologies to healthcare professionals with higher and secondary medical or pharmaceutical education. The paper analyzes these problems and proposes possible ways to solve them. The methodological basis of the work was the integration and generalization of previously conducted research on increasing the effectiveness and individualization of education. The paper examines the features of the organization of the educational process in additional professional education and shows the need to use elements of distance learning to form individual paths. The tasks that can be solved using a distance learning system during full-time education are analyzed. Methods for individual paths development are proposed for cases when students can only partially attend classes and cases where it is necessary to compensate for differences in the levels of initial schooling of students.

PERCEPTIONS OF ACADEMIC INTEGRITY IN PREPARATION FOR CLASS

Abstract

Background. In the modern educational process, methods of knowledge assessment and students’ motivation play a key role in the effectiveness of learning. Studying the perception of academic honesty and methods of preparation for practical classes can provide important data on the current state of the educational process.

The study aimed to investigate of the attitude of students and teachers to the principles of academic policy in preparation for practical classes, determination of conscientiousness, and identification of factors stimulating their motivation to study in preparation for practical classes and to identify the factors stimulating their motivation to study.

Material and methods. An online survey was conducted among 610 respondents, including students and teachers from various medical specialties. The survey covered questions regarding the most effective methods, motivational factors, the use of additional literature, and the perception of academic integrity policy.

Results. Traditional forms of assessment remain relevant for 36.2% of respondents, 29% consider the demonstration of practical application of knowledge to be the most indicative in the profession. About 16.4% of respondents prefer self-assessment, and their motivation for learning is determined by interest in the profession, personal goals, value of the specialty, and career prospects. More than 50% of students actively use additional literature, which emphasizes the lack of satisfaction with basic sources and more than 90% consider a commitment to academic integrity and honesty.

Discussion. The heterogeneity of responses in the perception of academic honesty, learning styles, and awareness of professional interests indicates the need for additional efforts by the university to raise students’ awareness of the consequences of violations and strengthen their understanding of academic honesty, as well as the possibility that students can expect support and motivation to achieve their learning outcomes.

Conclusion. To improve the educational process effectiveness necessary to adapt teaching and assessment methods to consider the diversity of students’ needs and motivations, strengthen the idea of recognizing the policy of academic integrity, and develop the practical application of knowledge and opportunities for self-assessment in the curriculum.

ANXIETY OF DOCTORS-TEACHERS OF HIGHER EDUCATION AS A RISK FACTOR FOR PROFESSIONAL DISADAPTATION

Abstract

Professional adaptation characteristics are an important aspect of every medical worker activity. The ability to withstand various professional stress factors, maintaining a stable emotional background and using adaptive forms of behavior, allows a physician to be professionally successful, providing assistance to a patient in today’s difficult conditions. Of particular interest is the consideration of this problem by specialists who combine the activities of a physician and a teacher at a higher medical educational institution. The need to combine different professional models requires flexible adaptation in different work situations, maintaining a stable emotional state, and confidence in one’s own professionalism. The presence of a state of severe anxiety can be considered as a predictor of professional maladaptation.

Research results of medical university teachers are presented in this article. Data analysis showed that specialists with severe anxiety are characterized by: concentration on negative aspects of professional activity, emotional experiences, low assessment of life quality, tendency to develop addictions. In general, these specialists can be characterized as a risk group susceptible to professional maladaptation.

The data obtained indicate the need to develop organizational and psycho-preventive measures aimed at increasing the adaptive capabilities of this contingent, as well as further study of this issue.

DIGITAL TRANSFORMATION OF A MEDICAL UNIVERSITY ON THE EXAMPLE OF KURSK STATE MEDICAL UNIVERSITY

Abstract

Relevance. The training of medical specialists has traditionally been one of the conservative and inert spheres of the education system not only in Russia, but in the world as a whole, but the digital transformation of the social and economic sphere dictates the need for digital transformation of the entire higher education system. Currently, there are many different sets of indicators aimed at an objective assessment of the effectiveness of targeted programs and projects in the field of digital transformation in the field of higher education, but there is no universally recognized one.

The purpose of the study is to determine, based on the analysis of the literature, an objective and simple way to assess digital maturity for an educational organization of higher education and, using the example of an industry regional medical university, to analyze some of the most important criteria for assessing digital transformation and determine the level of digital maturity of Kursk State Medical University in comparison with other industry universities.

Materials and methods. The assessment of digital maturity was carried out in accordance with the methodology developed by the Moscow Institute of Physics and Technology (National Research University) for 5 tracks: “Digital Services”, “Information Systems”, “Data Management”, “Infrastructure”, “Personnel”.

Results. According to the results of assessing the digital maturity of the Kursk State Medical University, it was found that the university by a total score of 86 out of 100 practically reached digital maturity, while the largest number of points scored on the track “Information Services” – 100 points, and the smallest – 70 points on the track “Infrastructure”, taking 6th place among 46 industry medical universities.

ON-SITE INDUSTRIAL PRACTICE AS A CONDITION FOR THE PROFESSIONAL DETERMINATION OF A RESIDENT

Abstract

Modern training of qualified medical personnel in conjunction with the documents regulating the implementation of educational programs of higher medical education makes it possible to significantly individualize the learning process, which increases the level of knowledge and skills of specialists entering practical healthcare. An individual approach in the practical training of residents also contributes to early professional self-determination, the main factors of which are: self-esteem, formed value orientation, the presence of internal motivation for choice, personal career planning, the level of requirements and the development of universal and professional abilities. The aim of this article is to summarize the experience of Krasnoyarsk State Medical University in the formation of an individual trajectory of practical training for residents, which contributes to the early professional self-determination of students and assistance in their employment in their chosen specialty in medical organizations of the Krasnoyarsk region.
