«DECISION COACHER»: CONTINUOUS PROFESSIONAL EDUCATION PROGRAM DEVELOPMENT
AbstractShared decision–making is one of the criteria for the quality of medical care abroad. Professional patients’ coaching by healthcare specialists allows to increase patients’ engagement, commitment and treatment satisfaction. Training programs for «patient mentors» are included in residency courses and additional professional education abroad. At the moment, there are no such programs in Russia, as well as the practice of shared decision-making.
The aim – to analyze educational programs, evaluate methodological approaches, develop a program of additional professional education, adapting it to Russian practice.
Material and methods. Historical method, analysis of foreign standards and protocols, content analysis of literature in PubMed. Based on the synthesis of information, foreign practices, key provisions of the educational program were developed. The professional standard «nursing» was used as a base.
Results. There are four models of decision-making coaching guidelines. The results of educational coaching programs can be evaluated indirectly by patients’ engagement, increasing their level of responsibility. Nursing staff are preferable as a patient coacher as it helps to increase their social status and reduce the burden on a doctor. Mandatory blocks of all programs are: patient’s narratives, study of models of decision-making, development of communication skills, principles of familiarizing patients with the evidence-based approach in medicine, assessment of patients’ willingness to participate in the choice of a treatment method, use of decision support tools.
Conclusion. The training of «patient mentors» is a new vector for the development of continuous nursing education. The approach of shared decision-making on treatment is currently not implemented in Russian medical practice due to the lack of standards. The development of a program of additional professional education for secondary medical stuff makes it possible to take the first step towards patient-centered approaches of “doctor–patient” interaction.

Основы доказательной медицины. В рамках данного блока слушателям даются основы доказательной медицины - методы поиска научной информации, ее критическая оценка и использование для принятия рациональных клинических решений.
Ознакомление пациента с доказательным подходом в медицине. Цель данного раздела - научить слушателей знакомить пациента в простой и понятной ему форме с рисками и преимуществами метода лечения, критической оценке и помочь пациентам принимать решение с учетом медицинских факторов.
Оценка готовности пациентов участвовать в выборе метода лечения. Данный раздел учит слушателей мотивировать пациентов на активную позицию при выборе метода лечения. В рамках блока разбираются инструменты оценки вовлеченности пациента в выборе лечения.
Использование средств поддержки принятия решения в практике коуча и разработка средств поддержки принятия решений пациентом представляет собой блок программы, целью которого является обучение сбору и систематизации информации для пациента, основанной на фактических данных, и представление ее всеобъемлющим, понятным, прозрачным и нейтральным образом. Инструменты поддержки принятия решений представляют собой брошюры или другие информационные материалы на бумажном или электронном носителе, которые ясно описывают варианты лечения, ухода и реабилитации - терапевтические траектории. Они предоставляют основанную на фактических данных информацию о связанных с каждой терапевтической траекторией преимуществах и недостатках, помогая пациентам рассмотреть различные аспекты выбора лечения, включая отказ от лечения. Средства поддержки принятия решений разрабатываются на основе локальных клинических рекомендаций и с учетом международных стандартов разработки подобных материалов [28].
Таким образом, на основании изученной литературы и зарубежного опыта организации образовательных программ по направлению коучинга пациентов была разработана и предложена учебная программа повышения квалификации "Наставник пациента" общей трудоемкостью 18 академических часов (табл.).

Программа разработана на основе федерального государственного образовательного стандарта среднего профессионального образования по специальности 34.02.01 "Сестринское дело", утвержденного приказом Министерства образования и науки Российской Федерации № 502 от 12 мая 2014 г. Программа может быть реализована в гибридном (онлайн и офлайн) формате.
Цель реализации программы - приобретение обучающимся новых профессиональных компетенций, в частности:
ПК 1.1. Проводить мероприятия по сохранению и укреплению здоровья населения, пациента и его окружения.
ПК 1.2. Проводить санитарно-гигиеническое просвещение населения.
ПК 2.3. Сотрудничать с взаимодействующими организациями и службами.
ПК 4.4. Проводить исследовательскую работу по анализу и оценке качества сестринской помощи, способствовать внедрению современных медицинских технологий.
ПК 5.1. Организовывать и оказывать сестринскую помощь, консультировать по вопросам укрепления здоровья пациента, его семьи, в том числе детей; групп населения в учреждениях первичной медико-санитарной помощи.
Обсуждение
Программы подготовки наставников пациентов за рубежом закреплены в нормативных документах по соответствующему профилю. Так, в Германии подобные программы включены как часть Национального плана по борьбе с онкологическими заболеваниями, разработаны на основании закона о правах пациентов Германии и локальных клинических рекомендаций [29].
Изменение роли практикующих медсестер может оказаться достаточно сложным и потребовать особого внимания к их навыкам, умениям и ментальным установкам, а также учета особенностей функционирования медицинской организации. Внедрение практики совместного принятия решения о лечении требует не только пересмотра внутренних процедур принятия решений, взаимодействия с пациентом и его родственниками, но и разработки соответствующих стандартов с их нормативно-правовым закреплением на национальном уровне.
В то же время реализация подхода вовлечения пациента в выбор лечения решает проблему приверженности пациентов, а также позволяет, как показывают исследования, в долгосрочной перспективе снижать экономические расходы. Можно ожидать, что смена статуса практикующей медицинской сестры на коуча пациента позволит повысить статус медицинской сестры, ее роль в клинических процессах медицинской организации, увеличит мотивацию на саморазвитие.
Заключение
Актуальность разработки программы дополнительного профессионального образования "Наставник пациента" обусловлена общим трендом российского здравоохранения на пациентоориентированные подходы [30]. Коуч, или наставник, пациента является связующим звеном между пациентами и клиницистами, способствуя приверженности пациентов, развитию у них ответственности и в итоге к удовлетворенности результатом лечения [5-7].
Подготовка наставников пациента является новым вектором развития дополнительного сестринского образования. Подход совместного принятия решения о лечении в настоящий момент пока не в полной мере реализован в отечественной медицинской практике ввиду отсутствия стандартов. Разработка программы дополнительного профессионального образования для среднего медицинского персонала позволяет сделать первый шаг в сторону пациентоцентричных подходов взаимодействия "врач-пациент".
