ORGANIZATIONAL ASPECTS OF MULTIDISCIPLINARY TEAMS: QUALITY STANDARDS, EFFICIENCY AND IMPLEMENTATION IN PRACTICE

Abstract

The multidisciplinary approach is considered to be the gold standard in clinical decision-making and can be implemented at the organizational level as a multidisciplinary team, or at the functional level – as a clinical decision-making process. In Russia, there is no practice of assessing the quality and effectiveness of multidisciplinary teams, which requires familiarization with existing approaches to its assessment, and determining the necessary competencies of specialists.

The aim – to describe the existing approaches to assessing the quality and effectiveness of multidisciplinary teams, to provide recommendations on their implementation in local clinical practice and education.

Material and methods included the historical method, analysis of regulatory information, content analysis of literature.

Results. In Russia, the need for a multidisciplinary approach is fixed in the procedures for providing medical care and is implemented at the functional level. There are seven standards for evaluating the quality of MDT, of which TEAM is the most comprehensive. The most significant factors for the success of multidisciplinary teams are the group work of its members and organizational culture, the implementation of patient–centered principles and the clinical decision-making process, which comprehensively takes into account various aspects of medical care – clinical outcome and possible complications, tariffs within the medical insurance system, hospital expenses and available resources, individual preferences of the patient.

Conclusion. Healthcare managers should be aware of the importance of assessing the quality and effectiveness of MDT work and implement appropriate assessment standards in practice. Within the framework of continuous medical education programs, special attention should be paid to the development of «soft» skills among medical specialists – teamwork, patient-centricity, organizational behavior.

Большое внимание уделяется организационной культуре в команде. В частности NCAT рекомендует оценивать социальный климат в команде по следующим критериям:

· имеются взаимное уважение и доверие между членами команды;

· все участники имеют равный голос, и разные мнения ценятся;

· конфликты между членами команды разрешаются быстро и собственными силами;

· поощряется конструктивное обсуждение/дебаты;

· отсутствуют личные цели при принятии клинических решений;

· есть возможность запросить и предоставить разъяснения, если что-то неясно.

Все процессы: операционные, коммуникационные, процессы принятия клинических решений - должны быть задокументированы, доступны для ознакомления каждому члену команды, подлежат обновлению и усовершенствованию по мере необходимости, а также должны быть обеспечены соответствующими ресурсами (технологическими, временными, кадровыми и пр.).

Отдельно описаны пациентоориентированные критерии работы MDT.

· Пациенты осведомлены о целях и принципах работы MDT.

· Взгляды, предпочтения и ценностные потребности пациента представлены кем-то из членов команды.

· Руководитель МДК несет ответственность за назначение ключевого сотрудника, представляющего интересы пациента.

· Назначенное лицо несет ответственность за обеспечение того, чтобы информационные потребности пациента были оценены и удовлетворены.

· Пациентам предоставляется информация, соответствующая их пожеланиям, об их заболевании, диагнозе и вариантах лечения, включая методы лечения, которые могут быть доступны по направлению в другие организации, эта информация достаточная для принятия обоснованного выбора/решения об их лечении и уходе.

· MDT рассматривают все клинически подходящие варианты лечения для пациента, даже те, которые они не могут предложить/предоставить на месте.

· MDT имеют доступ к списку всех текущих и соответствующих клинических испытаний, включая критерии приемлемости, и рассматривают пригодность пациентов для соответствующих клинических испытаний в рамках процесса принятия решений.

· Всегда рассматриваются вопросы психосоциальной, поддерживающей и паллиативной помощи пациента (например, с помощью комплексной оценки потребностей).

· Мнения, предпочтения и потребности пациента влияют на процесс принятия решений, когда это уместно и возможно.

Результатом процесса принятия клинических решений MDT являются четкие рекомендации по плану лечения/ухода, полученные в результате встречи. Эти рекомендации должны быть:

· основаны на фактических данных;

· ориентированы на пациента (в соответствии с взглядами и предпочтениями пациента, когда они известны, и с учетом сопутствующих заболеваний);

· основываться на стандартах, протоколах лечения, локальных клинических рекомендациях, если нет веских причин против этого, которые затем должны быть задокументированы.

Обсуждение

Все применяемые за рубежом QAT-средства использовали для оценки шкалу Лайкерта, а объективные показатели результатов отсутствовали, что делает сравнение результатов (бенчмаркинг) затруднительным. Кроме того, в настоящее время отсутствуют исследования "случай-контроль", посвященные изучению влияния качества подготовки МДК на качество принимаемых клинических решений. Несмотря на упомянутые недостатки, главным преимуществом всех QAT-средств были простота в применении, удобство использования и низкая стоимость внедрения.

Заключение

Мультидисциплинарный подход может быть реализован как на организационном уровне в формате МДК, так и на функциональном - как процесс принятия клинического решения на медицинском консилиуме. Применение QAT-средств позволяет оценивать качество работы МДК и совершенствовать их эффективность. Наиболее значимыми факторами успеха работы МДК являются групповая работа его членов и организационная культура, реализация пациентоцентричных принципов и процесс принятия клинического решения, который всесторонне учитывает различные аспекты медицинской помощи - клинический результат и возможные осложнения метода лечения, тарифы в рамках системы медицинского страхования, расходы медицинской организации, имеющиеся в распоряжении ресурсы, индивидуальные предпочтения пациента.
