ASSESSMENT OF CLINICAL COMPETENCE USING OBJECTIVE STRUCTURED CONTROL BASED ON INTEGRATED MODULES

Abstract

Introduction. The Objective Structured Clinical Examination (OSCE) is an important valid mechanism for evaluating students' practical knowledge. In the Republic of Kazakhstan, the objective structured clinical examination (OSCE) is included as a mandatory component of the final control for students of intermediate and graduate courses of medical universities of the country.

Material and methods. The article traces the experience of conducting OSCE among 3rd year students of the specialty "General Medicine" of the Karaganda Medical University, Republic of Kazakhstan.

Results. The authors have shown that OSCE helps to demonstrate that students have mastered the following learning competencies in the discipline "Pathology of organs and systems": knowledge of biomedical sciences, communication skills, teamwork skills. The authors note that positive aspects of this teaching method include the ability to show students the integrated knowledge gained in the study of the discipline, while mastering skills, and teachers to objectively assess practical skills.

Conclusion. The practice of using an objective structured clinical examination (OSCE) in higher medical schools for assessing clinical competence in different countries of the world is more than 40 years. But the literature contains sporadic information about the procedure for passing practical skills in clinical disciplines in foreign medical universities, referred to as "an exam with a full description of clinical cases". Since 2014 in the Republic of Kazakhstan the OSCE has been a mandatory component of the final control for medical students studying under the bachelor's degree program in senior years and under the residency program. The difficulty lies in the integrated teaching and control of the development of clinical competence in 3rd-year students studying basic and mandatory disciplines in the pathology of eight systems: cardiovascular, respiratory, nervous, endocrine, digestive, urinary, haematological, musculo-skeletal.

AMEE GUIDE # 119. THE FOUNDATIONS OF MEASUREMENT AND ASSESSMENT IN MEDICAL EDUCATION

Abstract

As a medical educator, you may be directly or indirectly involved in the quality of assessments. Measurement has a substantial role in developing the quality of assessment questions and student learning. The information provided by psychometric data can improve pedagogical issues in medical education.

By measuring, we are able to assess the learning experiences of students. Standard setting plays an important role in assessing the performance quality of students as doctors in the future. Presentation of performance data for standard setters may contribute towards developing a credible and defensible pass mark. Validity and reliability of test scores are the most important factors for developing quality assessment questions. The analysis of assessment individual questions provide useful feedback for assessment leads in order to improve the quality of each question, and hence make students' marks fair in terms of the diversity and ethnicity. Item Characteristic Curves (ICC), Differential Item Function (DIF) analysis and option analysis will send signals to assessment leads to improve the quality of individual question.

ON THE ISSUE OF TEACHING MEDICAL PREVENTION AND FORMATION OF HEALTHY LIFESTYLE TO STUDENTS OF MEDICAL UNIVERSITIES

Abstract

Ensuring the competitiveness of Russian education in terms of the quality of teaching and the upbringing of a harmoniously developed, socially responsible person is the goal of the national project of the Russian Federation "Education" (2018). In accordance with this program document, when training practical health professionals, it is necessary to take into account the modern requirement of the time - to fully meet the needs of society in the implementation of comprehensive prevention of non-communicable diseases (NCDs). The article presents the theoretical and practical aspects of the implementation of psychological and pedagogical innovative technologies in the teaching of the discipline "Medical prevention and formation of healthy lifestyle", approaches to teaching medical students to identify and prevent risk factors for NCDs, the formation of competence to competently and convincingly argue the need to change the lifestyle to a healthier one, using effective communication methods.

DISTANCE LEARNING OPPORTUNITIES FOR TRAINING IN PRACTICAL SKILLS IN THE COVID-19 PANDEMIC

Abstract

Introduction. The analytical article presents the role of the Department of General Medical Practice of the Saint-Petersburg State Pediatric Medical University in the educational process in the development of practical skills.

Material and methods. The experience of teaching the discipline "Clinical teaching practice General care for sick adults and children of therapeutic and surgical profiles" obtained during the transition to distance learning during the spread of the new coronavirus infection COVID-19 is reflected.

Results. The adaptation of the pedagogical process through the use of interactive technologies in the teaching of students of the 1st and 2nd courses is aimed at the acquisition of professional competences, including practical skills. The requirements for which are determined by the standards of the federal State education standard.

Discussion. The basic principles and problems of remote technologies in the educational process have been formulated. The authors have highlighted problems that should help to define the location of integrated remote education technologies in the educational process of practical skills development.

Conclusions. The rapid introduction of distance learning allowed the training process to continue at an appropriate level and standardized approaches to study of all material. However, the acquisition of practical skills in a remote format is incomplete, which requires the study of the survival of students' knowledge and the use of additional simulation training in the educational process.

EVIDENCE-BASED APPROACH IN CREATING ASSESSMENT TOOLS FOR ACCREDITATION OF HEALTHCARE PROFESSIONALS

Abstract

Basic topics of evidence approach and its opportunity for creation of tools innovation are considered. The stages of evidence approach application in practice of tools construction for accreditation of health care specialists are analyzed. The results of situational items approbation demanding the arguments for answers choice are given.

IMPROVING THE TRAINING OF DENTISTS IN RESIDENCY THROUGH THEIR MOTIVATION FOR EDUCATIONAL ACTIVITIES

Abstract

Introduction. The modernization of medical education carried out in our country provides for the improvement of the educational process with different categories of students, with special attention being paid to the training of residents. The purpose of this study was to substantiate the effective directions of motivation of dentists to educational activities in the residency.

Material and methods. The study was conducted on the basis of Sechenov University using the methods of analytical, logical, statistical and questionnaire survey. 107 respondents from among residents and teaching staff were involved in the questionnaire survey using a random a random sampling technique.

Results and discussion. It is established that the priority role in the motivation of residents belongs to: improving the quality of the selection of applicants for training, activating the internal motives of dentists to master the training program in the residency, improving the professional qualities of the teaching staff, attaching the resident to the mentor, conducting classes by teachers with high emotional energy and creating the necessary conditions for the educational process.

Conclusion. The conducted research allowed us to establish that the motivation of dentists for educational activities is an effective direction that ensures the high-quality development of their residency program.

MODERNIZATION OF HIGHER MEDICAL EDUCATION THROUGH CAPACITY BUILDING OF TEACHERS

Abstract

The paper considers implementation of a project to modernize medical education at the Avicenna Tajik State Medical University through changing the development university's strategy and capacity building of the faculty. The leading trainers of the Karaganda Medical University (Kazakhstan) took part in the modernization of the educational process of the TSMU. A study was carried out to determine the impact of the Project's results on increasing the potential of the faculty staff of the TSMU.

IMPLEMENTATION OF THERAPEUTIC DISCIPLINES AND CLINICAL PRACTICE IN THE CONTEXT OF A NEW CORONAVIRUS INFECTION EPIDEMIC

Abstract

Material and methods. This article presents the results of evaluating the effectiveness of teaching medical university students before and during quarantine measures related to C O V ID -19 on the example of the Department of Internal Medicine. The comparison of the rating points was carried out on the basis of the analysis of the intermediate certification sheets for the therapeutic disciplines taught at the pediatric, dental and preventive medicine faculties.

Results. In offline and online modes the rating points of the intermediate certification were obtained: in the discipline «faculty therapy», the IV-year students of the Faculty of Pediatrics had an average score of 3.5 points in 2018/2019, and 3.82 points in 2019/2020. In the discipline «internal diseases» for students of the Faculty of Medicine and Prevention in the 2018/2019 academic year, the average score was 3.6, and in 2019/2020 - 3.84 points and in the discipline «internal diseases» for students of the Faculty of Dentistry - 3.83 and 4.1 points respectively. Aspects of the implementation of a number of remote technologies and information systems into the educational process are considered.

Conclusion. In the current epidemiological conditions it is difficult to focus exclusively on the contract work of students with a teacher, and it is appropriate to look for new forms of professional competence formation. For more effective assimilation of the material, various forms of independent work (audio and video materials, texts, hyperlinks) in metered volumes should be used. Along with real communication with patients, it is important to include more advanced online simulators in the educational process.

FEATURES OF PSYCHOEMOTIONAL STATE IN MEDICAL UNIVERSITY STUDENTS IN THE CONTEXT OF DISTANCE LEARNING

Abstract

Currently, in the context of the massive spread of the new coronavirus infection COVID-19 in the Russian Federation, distance learning has become the main form of education at universities.

Material and methods. During the period of distance learning in the spring semester of 2020, 60 students of a medical university assessed the level of anxiety and associated negative emotional experiences, stress and stress resistance, followed by a comparison of the data obtained by the same students during the period of traditional study at the university in the fall semester of 2019.

Results. A comparative analysis of the survey data of medical university students during the period of distance learning (spring 2020) and traditional education (autumn 2019) showed that during the pandemic, the number of students with a high level of anxiety increased by 1.9 times, with a high level of negative emotional experiences by 1.7 times, as well as an increase by 2.1 times in students with a high level of stress.

Discussion. The data obtained indicate the possibility of the formation of risks of reducing the academic performance of students and require the development of a monitoring system for the mental state of students and its correction, as well as the improvement of the regime and forms of the educational process in the new digital environment.

Conclusion. The experience gained in the implementation of distance learning in medical education testifies to some negative phenomena, which were characterized by an increase in the level of stress, anxiety and negative emotional experiences, which requires the educational institution of higher medical professional education to solve the problems of not only the curriculum with this training format, but also to create conditions for the prevention of the formation of psycho-emotional discomfort in students.

DISTANCE LEARNING THROUGH THE EYES OF RESIDENTS

Abstract

The aim of the study was to investigate the attitude of medical residents to introduction of distance learning during a pandemic caused by SARS-CoV-2 virus in the Russian Federation.

Material and methods. A statistical analysis of questionnaire responses of 37 first-and second-year pediatric residents, who responded to the questionnaire survey on topical issues of distance learning, was carried out. The survey was carried out on the basis of the Astrakhan State Medical University of the Ministry of Health of the Russian Federation.

Results. 83.8% of the respondents would not like to study remotely. 41.8% of respondents believe that distance learning (DL) will be relevant only during a pandemic, the rest of respondents assume that it will be possible to use world experience in diseases diagnosis and treatment through participation in international educational events and foreign literature study. 51.4% of residents would prefer full-time education, 40.5% of residents pointed that they want full-time/part-time education, and only 8.1% of residents want distance learning. Previously, respondents have already encountered distance learning and, based on acquired experience: 29.1% of respondents would prefer teleconferencing as one of educational forms; 19.4% of respondents would prefer communication via e-mail; and 18.4% of respondents would like to study through the chat rooms that will be created. Others preferred electronic libraries and electronic learning materials. 27.2% of residents noted that insufficient technical equipment of the educational process hinder distance learning at the university, 18.5% of residents consider the level of proficiency in information and communication technologies of faculty members to be insufficient, 17.3% of residents answered that there are difficulties in assessing learning outcomes and there are no specialized teaching materials, 12.3% of residents believe that existing problems are associated with a lack of scientific and methodological foundations, and only 7.4% of residents indicated that problems with distance learning are related to lack of legal and regulatory framework.

Conclusion. The presented results of the study allow us to conclude that majority of respondents consider distance learning as an auxiliary way of obtaining knowledge, preferring traditional training method. Taking into account results of the survey, organizers of educational process will be able to adjust presentation of educational material and improve the quality of distance learning.

Abstract

The article analyzes draft federal state educational standards for residency programs, provides their comparative characteristics with the current federal state educational standards. The planned modernization of the residency institute in Russia is discussed in terms of the timing of the development of residency programs in some specialties and the introduction of phased (discrete) training in residency.
