Experience of introduction of algoritlimized virtual patient in pedagogical practice on discipline "ophthalmology"

Abstract

Introduction. A distinctive feature of the ap­proach used is the application of the virtual patient concept. Instead of a clinical case, a special computer program is used on the pa­per during the lesson, allowing the learners to make decisions on the diagnosis and treat­ment of the patient and immediately see the results of their actions. "Variants are thus ob­tained branched and allow for a multiplicity of treatment options and diagnostics, includ­ing making mistakes by the students.

Aims — to develop a new method of forming clinical thinking among the 6th year students of the Karaganda State Medical University (KSMU), who study in the specialty "General Medicine", in the discipline "Ophthalmo­logy" using the algorithmized "Virtual Pa­tient" and evaluate its effectiveness.

Results. A new station for assessing clinical thinking about ophthalmology "Virtual pa­tient". The prospective, randomized study was conducted on the basis of the Department of Ophthalmology, Otorhinolaryngology and Reanimatology and the Center for Practi­cal Skills (CPS) of the KSMU from January to June 2016 to determine the effectiveness of the technology created in the discipline "Ophthalmology". The students of the 6th year who passed the exam at the new station were surveyed. The results are processed by statistical methods.

Conclusions. Using a virtual patient in the learning process allows you to approach the real situation, practice the skills of problem solving and decision-making, improve efficiency and productivity of training of both medical students and postgraduate students.

The use of the mental maps in the practice of a training of older year students in the discipline "children's infectious diseases"

Abstract
In the article an expediency of using the mental maps as an alternative to traditional educational literature for the training of the students of the older years for a practical training on the discipline "сhildren's infectious diseases" is studied. The purpose — to prove the advantages of using the mental maps before an educational student's literature. In the work concrete and sociological methods — voluntary anonymous questioning and the method of a simple interview with the purpose of s specification of some positions are used. 72 students of the 5th and the 6th courses of the general medicine and the pediatric faculties underwent questioning and interviewing. As a result of the conducted survey it turned out that the vast majority of the respondents (83.3%) in general positively estimated use of the mental tables by the preparation for a practical training in comparison with an educational literature. From the most significant positive features of use of this pedagogical reception students noted presentation of an information at its small volume, and its structure. Only 6.9% of the students showed a negative attitude to the mental maps, having indicated its insufficient completeness. Thus, the use of the mental maps as an alternative to educational literature in many cases will be expedient by preparation for a practical training on the discipline "Children's infectious diseases".

Computer model for solving classification problems in a software environment Rapid Miner

Abstract
The development of information technology and modern communications, the emergence of a large number of automated medical devices and computers in clinics has led to a significant increase in medical-specialized information and biomedical data. Methods and technologies data analysis Data Mining lets identify a number of hidden patterns and relationships between data of various nature. The decision rules obtained in the classification contain new medical knowledge. Statistical quality control of diagnostic tests with binary outcomes showed that a computer classification model can be used to construct diagnostic algorithms for diseases. The construction of model for solving classification problems in a software environment Rapid Miner in practical classes develops information and technological competencies, ensures the development of competences for making practical decisions in the real conditions of professional activity.

Unified state examination as a predictor of academic performance in medical university

Abstract
Currently the results of the unified state examination (USE) became the only criteria to be admitted to the majority of medical universities in Russia. However, this fact is still causing hot debates among medical education professionals concerning appropriateness of professional orientation and objective knowledge assessment of applicants as theyguarantee students' subsequent successful training at medical university. Moreover, an obvious trend to encourage additional financial resources from the Federal state budget and Sverdlovsk region budget for the target admission of applicants to the Ural State Medical University (USMU) makes us pay attention to the rating selection of future medical students. Recently we foundthe decrease of the average USE scoreof applicants to the general medicine faculty of USMU. The aim of research was to study association between USE scores and medical students' academic performance in the course of human anatomy.We analyzed data on academic performance of the students from general medicine faculty according to the rating system in the course ofhuman anatomy during 2014—2017 (total 716 student ratings) and compared their results to their USE score at admission to USMU. All students were divided into four groupsaccording to the source of financing the occupied training place at USMU. The comparable data were analyzed using standard statistical methods with rank correlation. The vast majority of compared groups showed positive correlation of the average strength with the correlation coefficient of 0.24 to 0.42. Thus, our research showed the association between USE scores and subsequent academic performance of medical students in the course of human anatomy..

Topical issues and the necessity for training of executives of medical organizations in the matters of disaster medicine in the continuing medical education system

Abstract
In reliance on peculiarities of the system of organization and provision of medical care to the persons injured in emergency situations the role of executives of medical organizations in accomplishment of the objectives of medical support for the population in emergency situations is determined. The issues of comprehensive provision of medical organizations preparedness for emergency response, among which training of the executive staff in actions in crisis situations prevails, are considered. In the course of renewal of vocational education and introduction of a new model of continuing medical and pharmaceutical education, the necessity in elaboration of special professional competencies in disaster medicine and professional standards, to train executives of medical organizations in elimination of the consequences of emergency situations, is justified. In view of professional experience of specialists of the All-Russian Centre for Disaster Medicine "Zaschita" the examples of professional competencies of executives of medical organizations in the issues of medical support for the population in emergency situations are provided. The main objectives for the development of the uniform requirements for educational materials and activities with regard to training of executives of medical organizations in the disaster medicine issues are defined.

Prospects of neurosurgical staff accreditation

Abstract

Aim — this article analyzes the problems as­sociated with the forthcoming introduction of neurosurgical staff accreditation.

Methods. The result of the search in the da­tabase PubMed were 23 publications focuses on continuing medical education and neuro-surgical simulation training. The analysis of the Russian legislation concerning issues of accreditation of doctors.

Results. An analysis of the literature data al­lowed us to determine the state of the prob­lem of continuing medical education of doc­tors and training neurosurgeons-residents in Europe and the United States, to make pro­posals for the organization of neurosurgeons accreditation system in Russia.

Conclusions. Organization of neurosurgeons accreditation requires discuss issues related to the definition of criteria for evaluating the practice of doctors and technical opportunity of examination on practical skills in simulat­ed conditions.

Improving quality of care via continuous medical education refinement

Abstract
Along with transition to a new system of continuing medical education (CME) in Russia, there is a need to reform the organization of educational process as well as the guidelines for submitting materials and training instructors. Doctors highly motivated for self-education should be constantly improving their knowledge through CME. This will lead to an improvement of medical care quality.

Modern criteria for assessing the quality of management training under the conditions of the postgraduate training reformation

Abstract
The formation of a new model of continuous medical education leads to high competition between employers of educational services. Thus, the content of the educational process must be effective, available and of high quality. It should be noted that to ensure availability and quality of the medical care highly qualified medical personnel is needed. Thus, the research was aimed at studying and analyzing the need for methods and forms of educational programs for management personnel in health care, to determine the main evaluation criteria for the quality of education. By questioning the respondents, the analysis of the educational process was carried out by the service producer, the direct consumer and the final user of the services, the head of the medical organization. As a result of the research, it was found that for the optimal transition to the system of continuous postgraduate training it is necessary to introduce the organizational form of the educational process with the element of distant and practical training on the bases of medical organizations, meeting the priority interests of practicing doctors.
The proposed adapted method of the situational analysis makes it possible to determine the most promising areas of activity in the field of providing educational services on the basis of identifying advantages and disadvantages, opportunities and threats, with their subsequent comprehensive analysis: to determine the need in lectures and practical classes, the presentation of the material, priority requests to teachers, as the suppliers of services.
